Terms and Conditions
1. Membership of the scheme is free.
2. Only one card will be issued per individual.
3. The card is only for the use of the cardholder, it is not transferable.
4. The card is the responsibility of the cardholder, lost, stolen or damaged cards will be replaced on payment of applicable charge (at the managers discretion, contact the Helpline on 233 3073 for more information).
5. It is the responsibility of the cardholder to book a place with the activity provider where stipulated.
6. The card is not valid if altered or defaced in anyway.
7. Liverpool City Council reserves the right to withdraw / amend the card. The card is the property of Liverpool City Council and must be returned upon request.
8. The card can only be used according to the terms and conditions laid down by the individual services offered with the 08Plus scheme.
9. The card must be produced before any activity is undertaken.
10. Liverpool City Council will not be liable if: a) the card is not honoured by any participating activity provider b) for any subsequent withdrawal of the scheme or the card.
11. Any changes to personal details, name, address, entitlements, must be notified to the 08Plus Helpline: 233 3073. Proof of new details will be required for update on card.
12. All personal data collected and used to produce the card is in accordance with the Data Protection Act 1998 and will not be made available to third parties (i.e. parties not part of the scheme) without your permission, unless in the interest of the detection of fraud or the detection of other illegal purposes.
